Caregivers are how medical cannabis actually reaches most patients
For minors, the elderly, the homebound, or anyone who can't make a dispensary trip themselves, caregivers are the program. Here's what every state means by it.
A medical marijuana caregiver is a state-registered, background-checked person who can legally purchase, transport, possess, and (in some states) cultivate medical cannabis on behalf of a registered patient. The patient designates the caregiver. The state approves the caregiver. From that point on, the caregiver picks up medication at a licensed dispensary — same as the patient would — using their own caregiver ID card.
In practice, caregivers are the delivery system of medical cannabis programs. A 9-year-old with seizures can't drive to a dispensary. A 78-year-old with neuropathy may not want to. A working parent caring for a non-verbal adult child can't pause life every two weeks for a pharmacy run. Caregivers fill that gap legally, with full state protection.
Every state Green Bridge Society serves has its own caregiver program — different fees, different patient limits, different background-check rules. The information below covers all of them. Pennsylvania is detailed first because it's our largest patient base, but the same Green Bridge intake form at the bottom of this page works for any state.
Who can become a caregiver?
In every state we serve, you do not need to be related to the patient. Anyone who meets the age and background-check requirements can register.
Parent or Legal Guardian
Parents and legal guardians of minor patients. Minors always require a designated caregiver in order to receive medication — every state Green Bridge serves.
Spouse or Partner
Spouses and domestic partners of adult patients who need help picking up their medication or managing their state account.
Family or Trusted Friend
Any trusted person 21 or older (18+ in some states). You do not need to be a blood relative. The patient designates you in their state portal.
Support for Homebound Adults
Adults who are elderly, disabled, nonverbal, or homebound may have a caregiver register and manage the program on their behalf.
Patient and caregiver limits vary by state. Pennsylvania allows unlimited patients per caregiver. Michigan, West Virginia, and Maryland cap caregivers at 5 patients. Ohio caps at 2 per caregiver. See the state-by-state breakdown below for current rules in each state.

Types you'll see in the PA portal

Adult Patient (Already Registered)
The patient already has a PA medical marijuana card and needs you added as their caregiver.
Minor / Homebound / Assistance Patient
The patient does not have their own ID card. You'll manage the account and pickups on their behalf.
Registering a New Patient
The patient hasn't registered yet. You'll register both yourself as caregiver and them as a new patient simultaneously.

Pennsylvania — how to link caregiver ↔ patient
Once you're approved, you need to connect your caregiver account to the patient's profile in the state portal.
From the patient's account: Log in to the PA DOH portal → select Manage Caregivers → Add Caregiver → enter the caregiver's name and ID number.

WV
West Virginia
5 patients · $50 · Hardship waiver
How it works
West Virginia caregivers register through the MyComplia portal. You must be 21+, a WV resident, and pass a criminal history background check. The Office of Medical Cannabis (OMC) reviews applications within ~30 days.
What's different about WV
- Financial hardship waiver available for the $50 fee — same as the patient program (income below 200% of the federal poverty level).
- Patient must already be approved by the Office of Medical Cannabis before the caregiver can register — except for minor patients, where the caregiver application must be ready before the minor is approved.
- Digital caregiver ID card — like WV patient cards, your caregiver ID is downloaded from MyComplia, not mailed.
- No home cultivation — caregivers must purchase from licensed dispensaries.
OH
Ohio

How it works
The patient's certifying physician (CTR — Certificate to Recommend) adds you as their caregiver in the Ohio Medical Marijuana Registry. You receive an activation email, finish your profile, pay the $25 fee, and the Division of Cannabis Control runs a background check. No fingerprinting required.
What's different about OH
- Doctor must add you — Ohio caregivers are added by the recommending physician, not by the patient directly. The patient asks the doctor to include you.
- Cannot be both a patient and an active caregiver simultaneously providing caregiver services for someone else.
- Veterans get 50% off the registration fee.
- No cultivation — Ohio doesn't permit home grow for medical patients or caregivers.
MI
Michigan

How it works
Michigan is unique — it's one of the few states we serve that allows caregivers to cultivate cannabis on behalf of their patients. Caregivers register through the Michigan Cannabis Regulatory Agency. You're designated when the patient applies (or amended onto an existing patient via the Add/Change Caregiver Form).
What's different about MI
- Cultivation rights — up to 12 plants per patient, max 60 plants serving 5 patients (plus 12 of your own if you're also a registered patient, for up to 72).
- Possession — up to 2.5 oz per patient.
- Disqualifying record — drug or violent felony ever; other felonies within the past 10 years.
- One primary caregiver per patient — Michigan only allows a single primary caregiver per patient (unlike most states that allow 2).
- 2-year card validity — longer than most states.
VA
Virginia
"Registered Agent" · $25 · No background check
How it works
Virginia uses different terminology — what other states call a "caregiver" is called a "registered agent" in Virginia. There's no formal background check. The patient asks their certifying practitioner to include the agent's information on the written certification at the time of issuance. If added afterward through the CCA Portal, a $25 fee applies (current sources indicate the $25 fee applies in both cases for the registration itself).
What's different about VA
- One of the simplest in the country — no formal application or background check.
- Up to 2 patients per registered agent.
- Minor/incapacitated adult patients can list up to two parents/legal guardians plus one registered agent.
- Add at the appointment — easiest path. Tell the doctor you want a registered agent before the certification is finalized.
MD
Maryland
5 patients · FREE · 6-year card
How it works
Maryland caregivers register at the Maryland Cannabis Administration OneStop portal. Application is free. The most common reason for rejection is unclear ID photos — make sure your photo meets passport-style guidelines. Application processing can take up to 30 days.
What's different about MD
- Caregiver registers first, then the patient designates the caregiver from their OneStop account afterward.
- Up to 2 caregivers per patient; minors are required to have at least one (parents/legal guardians serve as a minor's caregiver).
- 6-year card validity — far longer than most states.
- Home delivery allowed — Maryland allows medical patients and their caregivers to receive cannabis via home delivery from licensed dispensaries.
OK
Oklahoma
Homebound & minors · Physician-justified
How it works
Oklahoma caregivers register through the OMMA MedPortal. Unlike most states, the physician must certify a medical need for a caregiver on the Physician Recommendation Form — minor status alone is not enough. Both caregiver and patient sign a Caregiver Designation Form to establish the relationship.
What's different about OK
- Physician must justify the caregiver — they sign off that the patient medically needs one.
- Eligibility limited to homebound patients by current OMMA guidance.
- Caregiver license tied to patient expiration — your caregiver license expires when the patient's license expires (up to 2 years).
- Minimum age is 18 for caregivers (lower than most states).
NY
New York

How it works
New York permanently waived patient and caregiver registration fees. Patients are auto-registered when their healthcare provider issues a certification. After registration, the patient can add up to 5 designated caregivers through the NY OCM patient system. Each caregiver gets their own digital credentials.
What's different about NY
- Most generous in the country — up to 5 caregivers per patient (vs. 1–2 in most states).
- No application fee for patient or caregiver since 2022.
- Home cultivation allowed — certified patients and designated caregivers 21+ may cultivate cannabis at home for the patient's personal use.
- Caregiver facilities — NY allows hospitals, residential facilities, and schools to register as designated caregiver facilities, useful for institutional patients.
LA
Louisiana
No formal registry · Pharmacy model · Minors
How it works
Louisiana doesn't have a traditional caregiver registry like other states. The therapeutic cannabis program works through licensed pharmacies (not dispensaries). The physician's recommendation goes directly to the patient's chosen pharmacy. Adult patients don't need a separate caregiver designation — they go to the pharmacy directly.
What's different about LA
- Minor patients — parents or legal guardians serve as caregivers for minors; this is built into the pharmacy process, not a separate registration.
- Incapacitated adults may also have a parent/guardian or designated person serve as caregiver.
- Pharmacy model — caregivers contact the patient's pharmacy directly to ensure recommendations are processed correctly.
- No state-issued card — like the patient program, there's no caregiver ID card to obtain.
ME
Maine
Plant-based fees · Cultivation-focused
How it works
Maine treats caregivers more like small commercial producers. Caregivers who cultivate on behalf of patients register with the Maine Office of Cannabis Policy and pay tiered fees based on the number of plants they grow. Caregivers can cultivate up to 6 mature plants and 12 immature plants per patient, capped at 30 mature/60 immature plants total.
What's different about ME
- Caregiver = grower — Maine's caregiver model is built around home cultivation, not just pickup.
- Plant-tier fees — fees scale from $240 (up to 6 mature plants) to $1,200 (30 mature plants).
- Family exemption — caregivers who are parents/legal guardians of a minor patient, or who are growing for a household member, may not need to pay the annual registration fee.
- 1-year validity — caregiver IDs must be renewed annually.
State rules change. Fees, patient limits, and approval timelines are reviewed against official state sources as of May 2026, but states update their programs regularly.
