How to Get a Medical Cannabis Card in Minnesota
🏥 Two-Step Process
Getting your Minnesota medical cannabis certification involves two steps: seeing a registered Minnesota healthcare practitioner for certification, then enrolling in the Medical Cannabis Registry with the Office of Cannabis Management (OCM).
State of Minnesota (OCM) — manages the Medical Cannabis Registry, processes patient enrollments, and grants dispensary access.
Green Bridge Society — connects you with a Minnesota-registered practitioner who certifies your qualifying condition, then submits your certification electronically to the OCM.
💰 Why Get a Medical Card When Adult-Use Is Legal in MN?
Minnesota legalized adult-use cannabis in May 2023, but medical patients still get real advantages over recreational consumers:
✅ No state enrollment fee — Minnesota eliminated the $200 annual fee effective July 1, 2023
✅ 3-year certification — much longer than the typical 1-year cycle in most states
✅ Access at 18+ — recreational requires 21+
✅ Pharmacist-guided product selection — Minnesota dispensaries have on-staff pharmacists who help you choose products and dosing
✅ Higher possession limits for medical patients and specific patient-only product formats
📋 Minnesota's Patient Self-Evaluation Requirement
Minnesota has one rule that catches patients off guard: during your first year, you must complete a Patient Self-Evaluation (PSE) in the registry every 90 days. Dispensaries are not allowed to sell to you if your PSE is out of date.
Green Bridge Society automatically tracks your PSE deadlines and sends you reminders before each 90-day cycle — so you never lose dispensary access due to a missed form. This is part of every Minnesota certification we issue, at no extra cost.
📞 How Doctor Calls Work
Your Minnesota appointment is a telehealth visit by phone or video with a Minnesota-licensed practitioner. We call patients in the morning, afternoon, and evening — including weekends if scheduled on a Friday. You'll complete a short intake when you schedule.
Calls may come from a private, unknown, or spam-labeled number, so please keep your phone nearby and turned on.
🎖️ Minnesota Veterans: Self-Certification Available
Under Minnesota Statute 342.01 Subd. 63, veterans who receive care through the U.S. Department of Veterans Affairs can enroll in the Medical Cannabis Registry without seeing a healthcare practitioner. You'll need a Veteran Identification Card and an attestation that you've been diagnosed with one of Minnesota's 19 qualifying conditions.
Submit the Veteran Certification Form directly through the OCM registry. You'll receive a link to complete standard patient enrollment. No practitioner fee required.

About Minnesota's Medical Cannabis Program
Minnesota's medical cannabis program was created in 2014 under what was then Minnesota Statutes Chapter 152.22. The program was administered by the Department of Health's Office of Medical Cannabis from its launch through early 2025, when it transferred to the newly-established Office of Cannabis Management (OCM) under Minnesota Statutes Chapter 342. The transfer was part of Minnesota's 2023 cannabis law (HF 100), which legalized adult-use cannabis and consolidated medical and recreational oversight under a single agency.
Today the program is governed by Minnesota Statutes §342.51 through §342.60, with patient registry duties at §342.52, practitioner duties at §342.55, and qualifying conditions defined at §342.01 Subd. 63. Administrative rules are codified at Minnesota Rules Chapter 9810.
Minnesota's medical program serves tens of thousands of registered patients. Certifying healthcare practitioners — Minnesota-licensed physicians, advanced practice registered nurses (APRNs), and physician assistants — must register with the OCM before they can certify patients. Telehealth certifications are permitted under current OCM rules.
Patient enrollment in the registry is valid for 3 years. Re-enrollment begins 60 days before expiration. Veterans who receive care from the VA may self-certify without a practitioner visit. Minors can qualify with a designated caregiver (typically a parent or guardian).

What You'll Need for Your Minnesota Certification
📄 Before Your Appointment
To certify you for Minnesota's program, your practitioner needs to confirm a qualifying condition and establish a medical relationship.
✅ A valid Minnesota-issued photo ID (driver's license or state ID) — required for both certification and registry enrollment
✅ Medical documentation for your qualifying condition where available (recent records, diagnosis letter, or treatment history)
✅ A working phone number and email address — the OCM sends your registration link by email
✅ A list of current medications so your practitioner can advise on interactions
📋 After Your Certification — OCM Enrollment
Once your practitioner submits your certification, the OCM emails you a unique registration link. Complete the online enrollment with:
✅ Your unique OCM registration link (emailed to you)
✅ A government-issued Minnesota photo ID upload
✅ Basic personal and contact information
Minnesota Qualifying Conditions
Minnesota recognizes 19 qualifying medical conditions under §342.01 Subd. 63 for medical cannabis certification:
Note: Minnesota also recognizes patients whose qualifying condition or its treatment causes severe or chronic pain, severe nausea or vomiting, or cachexia/severe wasting — these symptoms expand the practical reach of the program. The OCM reviews petitions to add new conditions annually between June 1 and July 31, with decisions made by December 1.
Minnesota Rules to Know
📅 3-Year Registration
Minnesota patient enrollment is valid for 3 years from approval. Re-enrollment begins 60 days before expiration. This is one of the longest registration periods in the country — most states require annual renewal.
🔔 Patient Self-Evaluation Every 90 Days (Year 1)
During your first year of enrollment, you must complete a Patient Self-Evaluation in your registry account every 90 days. Dispensaries cannot sell to you if your PSE is out of date. Green Bridge sends automatic reminders before each cycle.
💊 Pharmacist Consultation Required Before First Purchase
Minnesota requires a consultation with a dispensary pharmacist before your first purchase at a dispensary. This is a state requirement, not a Green Bridge fee — the consultation happens at the dispensary and is part of the standard intake process. The pharmacist helps you select products, dosing, and form (flower, edibles, capsules, vape, topicals, etc.) based on your registry information.
⏳ 60-Day Enrollment Window
After your practitioner certifies you, you have 60 days (90 days maximum) to complete OCM registry enrollment. If you miss the window, your certification expires and you need to start over.
🪪 No Physical Card Issued
Minnesota does not issue a physical or digital medical cannabis card. Once approved, your name appears in the OCM registry. Dispensaries verify your status by registry lookup, not by a card. Bring your Minnesota photo ID to your first purchase.
Minnesota Medical Cannabis — Common Questions
Do I need to register with the state before seeing a doctor?
No. Minnesota does not require state pre-registration before certification. Your practitioner submits the certification directly to the OCM. After certification, the OCM emails you a unique registration link to complete your patient enrollment online.
How long does it take to get approved?
The OCM processes applications within 30 days, but many complete applications are approved within 72 hours. You'll receive an approval email when your registry account is active.
Is there a state fee?
No. Minnesota eliminated the $200 annual state enrollment fee effective July 1, 2023. The $149 certification fee covers your practitioner visit. Some dispensary partner programs may offer additional discounts — ask your dispensary or call us.
What is the Patient Self-Evaluation (PSE)?
The PSE is a short form you complete in your registry account every 90 days during your first year of enrollment. It asks about symptoms and whether the medicine is working. Dispensaries cannot sell to you if your PSE is out of date. Green Bridge sends automatic reminders before each cycle so you never lapse.
Is telehealth certification legal in Minnesota?
Yes. Telehealth certifications are permitted under current OCM rules (Minnesota Rules Chapter 9810). Your practitioner must be a Minnesota-licensed physician, advanced practice registered nurse, or physician assistant, registered with the OCM.
I'm a veteran. Do I still need a practitioner certification?
No — Minnesota veterans who receive care from the VA can self-certify using the OCM's Veteran Certification Form. You'll need a Veteran Identification Card and an attestation that you've been diagnosed with one of Minnesota's 19 qualifying conditions. Submit the form directly through the OCM registry and you'll receive an enrollment link by email.
What if I miss the doctor's call?
No problem. Your doctor will try again and may text you. Please keep your ringer on during your window. Calls can appear as "Unknown" or "Spam Risk."
Why get a medical card if adult-use cannabis is legal in Minnesota?
Several advantages: medical patients qualify at age 18+ (recreational requires 21+), keep their 3-year registration intact, get pharmacist-guided product selection at dispensaries, and have access to medical-specific product formats and higher possession limits. The Minnesota medical program also predates the recreational rollout and continues to be supported by the state.
How long does my Minnesota certification last?
Patient enrollment is valid for 3 years from OCM approval. Re-enrollment opens 60 days before expiration. This is significantly longer than most state programs.
What if I don't have medical documentation?
Your practitioner can evaluate your condition during the appointment based on your history and symptoms. Having documentation speeds things up, but it isn't always required — call us if you're unsure whether you have what you need.
What's the pharmacist consultation at the dispensary?
Minnesota requires a one-time consultation with the dispensary's on-staff pharmacist before your first purchase at that dispensary. The pharmacist reviews your registry information, talks through what you're using cannabis for, and recommends products and dosing. This is a state requirement built into Minnesota's program — it's part of the dispensary visit, not an extra Green Bridge fee.
Can I use my Minnesota card in other states?
Some states have reciprocity that may recognize Minnesota certifications for visiting medical patients — check the specific state's rules. Green Bridge Society also certifies patients in Pennsylvania, Ohio, West Virginia, Virginia, Maryland, Michigan, Oklahoma, New York, Louisiana, and Maine. See our FAQ page for general information across all states.
