How to Get a Medical Cannabis Card in Maryland
🏥 Two-Step Process
Getting your Maryland medical cannabis certification involves two steps: completing your Maryland state registration with the Maryland Cannabis Administration (MCA) and then seeing a certifying provider.
State of Maryland (MCA) — manages patient registration through OneStop and issues your MCA Patient ID Number.
Green Bridge Society — helps guide you through the process and connects you with a certifying provider once your Maryland registration is complete.
💰 Why Get a Medical Card When Adult-Use Is Legal in MD?
Maryland legalized adult-use cannabis in 2023 (retail sales began July 1), but medical patients still get real advantages over recreational consumers:
✅ No 9% sales tax on medical purchases — adult-use buyers pay it on every transaction
✅ Higher purchase limits — 120g per 30 days vs. recreational's 1.5oz cap per transaction
✅ Access at 18+ — recreational requires 21+
✅ More product variety — medical-only products like high-dose edibles and concentrates
📋 Maryland Requires State Registration First
Maryland patients should complete their state registration in OneStop before scheduling with a certifying provider. Once approved, you'll receive an MCA Patient ID Number, which is needed for certification.
Need help with the portal? Green Bridge can assist you with the Maryland state registration process, document questions, and general troubleshooting if needed.
📞 How Doctor Calls Work
Our doctors call patients in the morning, afternoon, and evening — including weekends if scheduled on a Friday. You'll complete a short intake when you schedule.
Calls may come from a private, unknown, or spam-labeled number, so please keep your phone nearby and turned on.
About Maryland's Medical Cannabis Program
Maryland's medical cannabis program was created in 2014 with the establishment of the Natalie M. LaPrade Maryland Medical Cannabis Commission (MMCC), named in memory of Delegate Cheryl Glenn's mother. The first medical cannabis sales began in December 2017. In 2023, the agency was renamed the Maryland Cannabis Administration (MCA) and expanded to oversee both medical and adult-use programs.
Adult-use cannabis was legalized through Question 4, a constitutional amendment approved by Maryland voters in November 2022, with retail sales launching July 1, 2023. The program is governed by Maryland's Health-General Article §13-3301 et seq., with regulations codified at COMAR Title 10, Subtitle 62.
Maryland's medical program serves over 100,000 active patients. Certifying providers — including physicians, nurse practitioners, dentists, podiatrists, and nurse midwives — must register with the MCA before they can certify patients. Telehealth certifications are explicitly permitted under Maryland law.
Patient registration is valid for 6 years. Provider certification must be renewed annually. The MCA may add new qualifying conditions if (1) the condition is debilitating, (2) cannabis can reasonably be expected to relieve it, and (3) other treatments have proven ineffective (per COMAR 10.62.07.06).

What You'll Need for Maryland OneStop Registration
📄 Registration Requirements
Maryland requires patients to register through OneStop before certification. Because registration requires sensitive personal information, including your full 9-digit Social Security Number, we recommend that patients complete this step themselves whenever possible.
✅ A valid, accessible email account
✅ Your full 9-digit SSN (entered directly in OneStop)
✅ A valid government-issued photo ID (MD driver's license, state ID, military ID, or passport)
✅ A recent passport-style photo (white/off-white background, square, taken within 6 months)
✅ Proof of Maryland residency (dated within 90 days)
📸 Photo Requirements (Common Rejection Reason)
MCA is strict about photos. To avoid your application being returned:
✅ File type: .jpg, .png, or .gif only
✅ Max size: 3 MB
✅ Square image: 600×600 px minimum, 1200×1200 px maximum
✅ Plain white or off-white background
✅ Taken within the last 6 months
❌ No hats, sunglasses, filters, or heavy shadows
Maryland Qualifying Conditions
Maryland recognizes the following qualifying medical conditions for medical cannabis certification:
Plus the catch-all clause: Maryland law also covers "any other chronic medical condition which is severe and for which other treatments have been ineffective." This is a powerful provision — many patients qualify under this language even if their specific condition isn't listed.
Maryland Rules to Know
⏳ 120-Day First Purchase Rule
If your certification is not used to make a purchase at a Maryland dispensary within 120 days, it becomes void.

Maryland tracks purchases on a rolling 30-day basis. Patients may purchase up to 120g of flower or the equivalent within that window.
🪪 Physical ID Card Is Optional

Doctor certification must be renewed annually.
Maryland Medical Cannabis — Common Questions
Do I need to register with the state before seeing a doctor?
Maryland patients should complete state registration through the MCA OneStop portal first. Once approved, you'll receive your MCA Patient ID Number, which is needed for certification.
How long does MCA registration take?
Processing times vary. Some patients are approved in 1-2 business days, while others may take longer depending on document review and application volume.
Do you help with the state portal?
Yes. If you need help with Maryland OneStop registration, document questions, or portal troubleshooting, our team can assist you. Because the state registration requires sensitive information including your full Social Security Number, we recommend that patients complete that step themselves whenever possible.
Can I use my certification right away?
Once your registration is active and your provider issues the certification, you can visit a licensed Maryland dispensary with your government-issued ID and your active Maryland patient information. Note: certifications must be used to make a purchase within 120 days or they become void.
What if I miss the doctor's call?
No problem. Your doctor will try again and may text you. Please keep your ringer on during your window. Calls can appear as "Unknown" or "Spam Risk."
What's the difference between the state fee and the doctor fee?
The doctor fee ($125 new / $75 renewal / $200 minor) covers your certification visit. Maryland also offers an optional physical patient card for $25 through OneStop. Patients can print a paper ID card at no charge. The $25 fee is waived for minors, hospice patients, and patients enrolled in the Maryland Medical Assistance Program or Veterans Affairs Maryland Health Care System.
What if I don't have medical documentation?
No problem — we offer an add-on evaluation for $75 where the doctor can evaluate and document your condition during the appointment. You can add this when you schedule.
Why get a medical card if adult-use cannabis is legal in Maryland?
Several major advantages: medical patients skip the 9% adult-use sales tax, can purchase up to 120g per 30 days (vs. recreational's 1.5oz per transaction cap), get access to medical-only high-dose products, and qualify at age 18+ instead of 21+. For regular consumers, the tax savings often pay for the certification within a few months.
Is telehealth certification legal in Maryland?
Yes. Maryland explicitly permits telehealth medical cannabis certifications. Your provider must be registered with the Maryland Cannabis Administration (MCA).
Can I use my Maryland card in other states?
Some states have reciprocity that may recognize Maryland certifications for visiting medical patients — check the specific state's rules. Green Bridge Society also certifies patients in Pennsylvania, Ohio, West Virginia, Virginia, Michigan, Oklahoma, New York, Louisiana, and Maine. See our FAQ page for general information across all states.
