✅ New Patient Certification – Michigan
Step 1: Create Your Online Account
If you don’t have a caregiver, you’ll need to create a secure online account through the Michigan Medical Marijuana Program (MMMP).

🔗 Login to the Michigan MMMP Portal
Step 2: Get Certified by a Physician
Your Green Bridge doctor will either:
Certify you through the state’s online portal OR
Give you a completed MMMP Physician Certification Form to upload.
🔄 Renewal Certification – Michigan
Step 1: Get Recertified
Your certifying doctor will either:
Certify you through the online portal, OR
Provide a new MMMP Physician Certification Form to upload.
Step 2: Log In and Start Renewal
Go to the MMMP Login Page
Click the “Registry Cards” tab
Choose “Renew Application”
Follow the prompts
Verify mailing address and upload proof of Michigan residency
Select your certification method (online or paper upload)
Pay the $40 state fee
Step 3: Apply for Your Patient Registry Card
Once logged in:
Click the “Registry Cards” tab
Select “Create an Application”
Follow the prompts
Verify your mailing address
Upload proof of Michigan residency:
Driver’s License Number, OR
State ID, OR
Voter registration + government-issued ID with name and DOB
Select whether your doctor will certify online or you will upload the form
Pay the $40 state fee
If your physician certifies you online, they must complete their part within 15 days of your application.
If you’re uploading a paper certification, the state has 15 business days to review.
