What You'll Need for Your OMMA Application

📄 Application Requirements

After your physician evaluation, you'll submit your application directly to OMMA. You'll need:

✅ A valid, accessible email account

✅ Your Adult Patient Recommendation form (signed by your physician — we provide this)

✅ A valid Oklahoma driver's license, state-issued ID, OR tribal ID

✅ A clear passport-style photo

Proof of Oklahoma residency (utility bill, rental agreement, mortgage statement, or other accepted document showing your current Oklahoma address)

✅ Application fee: $100, or $20 with Medicaid/Medicare/100% disabled veteran status

⚠️ Photo and ID must match. If you've recently moved to Oklahoma and your driver's license still shows another state, you'll need a current OK ID before applying. OMMA does not accept out-of-state ID for residency.

OMMA accepts standard passport-style photos. To avoid delays:

📊 Possession Limits

Licensed Oklahoma medical patients may possess:

• 3 ounces of marijuana on their person

• 8 ounces of marijuana in their residence

• 1 ounce of concentrated marijuana

• 72 ounces of edibles

• 6 mature plants and 6 seedlings (for personal cultivation at the patient's residence)

Once your OMMA card is approved, you can purchase from any of Oklahoma's licensed dispensaries — and there are more dispensaries per capita in Oklahoma than anywhere else in the United States.

🏠 Home Cultivation Allowed

Unlike most medical states, Oklahoma allows licensed patients to grow up to 6 mature plants and 6 seedlings at their residence for personal medical use.

No problem — we offer an add-on evaluation for $75 where the doctor can evaluate and document your condition during the appointment. You can add this when you schedule.
