How To Get A Medical Marijuana Certification

 

 

Step 1:


 
 

Qualify.

A Green Bridge certified physician will verify that you have a qualifying condition and make your profile in Ohio’s Patient & Caregiver Registry.* New patents are $200 and renewing patients are $150.

If you have not yet been diagnosed, but believe you are suffering from Post Traumatic Stress Disorder, please complete the PTSD Evaluation. Our doctors will review your form submission and determine a correct prognosis.

Step 2:


 
 

Register.

You will receive an email from no-reply-pmpaware@globalnotifications.com with a link to complete your registration through Ohio’s Patient & Caregiver Registry.** Complete your registration and pay the $50 Registration Fee to Ohio State.*** NOW ONLY A PENNY!!


Step 3:


 
 

Print Your Card.

After paying your registration fee and activating your card, you will be able to download and print out your Medical Marijuana card.


Step 4:


 
 

Dispensary.

We will then assist you in finding a state-licensed dispensary to obtain your medication and help prepare you for your trip to the dispensary.

  • AIDS

  • Alzheimer’s disease

  • Amyotrophic lateral sclerosis

  • Cachexia

  • Cancer

  • Chronic traumatic encephalopathy

  • Crohn’s disease

  • Epilepsy or another seizure disorder

  • Fibromyalgia

  • Glaucoma

  • Hepatitis C

  • Inflammatory bowel disease

  • Multiple sclerosis

  • Pain that is either chronic and severe or intractable

  • Parkinson’s disease

  • Positive status for HIV

  • Post-traumatic stress disorder

  • Sickle cell anemia

  • Spinal cord disease or injury

  • Tourette syndrome

  • Traumatic brain injury

  • Ulcerative colitis

Special Status

Those who qualify can have their $50 state registration reduced to $25. To receive this discount please let our physician know and please provide the appropriate documentation to the State of Ohio Board of Pharmacy. Documentation must identify you and include your date of birth.

To apply with Indigent Status please provide either:

  • Documentation that identifies you as being enrolled in the federal Social Security Disability Income program or the Supplemental Security Income program, along with the amount of money you are expected to receive during the year you apply to Ohio’s Medical Marijuana program.

  • Documentation that identifies you as being enrolled in a State Retirement System and the amount of disability benefits you are expected to receive the year you apply to Ohio’s Medical Marijuana program.

To apply with Veteran Status please provide one of the following:

  • A Department of Defense Identification Card.

  • DD214, DD215, or A National Guard Bureau military discharge certificate.

  • A report of separation from the National Archives National Personnel Records Center in St. Louis, Mo.

  • A Veterans Identification Card from the Department of Veterans Affairs.
