Renew your Ohio medical marijuana card online
One visit a year with your certifying physician, by phone or video, then two clicks in the registry. The state charges nothing — but the visit isn't skippable, and that's the part people miss.
Ohio eliminated the registration and renewal fee in May 2024. Once your physician has updated your recommendation, renewing in the registry costs nothing and takes about two minutes.
Two things have to happen
In this order. The registry won't renew a card without a current recommendation behind it.
Your annual recertification visit
Ohio requires at least one visit a year with your certifying physician to keep the physician-patient relationship active. Your physician then updates your recommendation in the registry.
$50 for returning patients, by telehealth.
Renewing the card itself
Log in to your registry account, confirm your details, and select the renew option. No fee, no forms, no documents to upload.
Free, and usually done in a couple of minutes.
How to renew
Start before your card expires — there's no grace period at the dispensary.
Book your recertification
Telehealth again, same as your first visit, and it's $50 for returning patients. Your physician reviews how the past year has gone and updates your recommendation in the registry.
Don't leave it to the day before. If your card lapses you can't purchase, and while getting back on track isn't difficult, the gap is avoidable.
Your physician updates the recommendation
This happens on the physician's side of the registry — nothing for you to file. Once it's in, your renewal option becomes available in your account.
Log in and select Renew Card
Sign in to the Ohio Medical Marijuana Registry, confirm your personal information is still correct, and select Renew Card. There's no payment step — if you're looking for one, you're not missing anything.
Update your address or email here if either has changed. This is the one moment each year you're guaranteed to be in the registry, so it's worth a look.
Your renewed digital card appears
The updated card is generally available in your registry account within a business day or two. Download it and replace the copy on your phone — the dispensary checks the current one.
What renewal costs
⚠️ "It's free, so why do I need an appointment?"
Because the free part is only the paperwork. Ohio law requires an annual visit with your certifying physician to maintain the bona fide physician-patient relationship, and the registry won't let you renew without a current recommendation on file.
Patients who hear "renewal is free" sometimes log in expecting to click a button and be done, then find the option isn't available to them. That's not a portal fault — it means the recommendation behind it has lapsed and needs the visit first.
Common questions
What renewing Ohio patients ask us.
I logged in and there's no renew option. Why?▾
Almost always because your recommendation has expired and hasn't been renewed by a physician yet. The renewal option appears once a current recommendation is on your profile. Book your recertification, and check back after the visit.
How early should I renew?▾
A few weeks before your expiry date is comfortable. Ohio's turnaround is fast — the card usually refreshes within one to two business days of renewing — so you don't need the long lead time some states demand. The appointment is the only thing that needs scheduling.
What happens if my card expires?▾
You can't purchase at a dispensary until it's renewed — Ohio requires an active registration and an active recommendation for anything to be dispensed. You don't lose your place in the program or start over as a new patient, though. Get recertified and renew, and you're back.
Can I switch to a different doctor at renewal?▾
Yes. You're not tied to whoever certified you last year — any Ohio physician with an active Certificate to Recommend can take over your recommendation. If you're coming to us from another practice, just book the renewal and mention it.
Do I have to re-upload documents or a photo?▾
No. Ohio's renewal is a confirmation, not a fresh application — you're checking that your existing details are right and accepting the terms. Update your address or email if they've changed, and that's the extent of it.
Does my caregiver need to renew too?▾
Yes, and theirs is free as well. A caregiver registration is tied to your patient registration, so if yours lapses theirs stops working too. Easiest to handle both at the same time — mention your caregiver at your recertification visit so the physician can keep the link current.
My card lapsed a long time ago. Am I renewing or starting fresh?▾
In most cases your registry profile is still there and you're renewing rather than starting over. Book the recertification and we'll see what the registry shows — if anything needs rebuilding on the state's side, we'll handle it from there. Either way it's the same visit.
Is it still worth keeping now that adult-use is legal?▾
For most regular purchasers, yes. Medical patients skip the 10% adult-use excise tax, get higher purchase and possession limits, and get medical pricing at most dispensaries. At $50 a year with no state fee, the tax saving alone usually covers it well before the year is out.
Time to renew?
Book your recertification first — the registry needs a current recommendation before it'll offer you the renewal. Telehealth, $50, nothing to pay the state.
