Becoming an Ohio medical marijuana patient
Ohio's process is one of the simplest we deal with — one telehealth visit, and the state's side is free. Here's how it works and why a card still matters now that adult-use is legal.
Ohio eliminated the patient and caregiver registration fee in May 2024. There is nothing to pay the state — not to register, and not to renew. If a service quotes you a state fee, that's out of date.
Your physician registers you — you can't sign yourself up
This is the part that confuses people who go looking for the application form. In Ohio there isn't one you can start on your own. Your physician has to hold a Certificate to Recommend (CTR) from the State Medical Board of Ohio, and it's the physician who creates your profile in the Medical Marijuana Registry using your ID details and email address.
Only after that does the registry email you to activate the account. So the order is always: see the doctor first, then finish your registration. There's nothing to do beforehand except book.
The three steps
Most Ohio patients are holding a digital card within a couple of days of their visit.
Book your telehealth visit
Pick a time and complete the intake form. You'll speak with an Ohio physician who holds an active CTR, by phone or video — no office visit needed.
Have your Ohio ID handy and give us an email address you can actually get into. The state sends your activation link there, and it's the one thing that can't be worked around later.
Your physician creates your registry profile
If you have a qualifying condition, your physician submits the recommendation and creates your profile in the Ohio Medical Marijuana Registry. If you want to add a caregiver, mention it at the visit — the physician is the one who links them to you.
The registry then emails you a confirmation with instructions for setting up your login.
Log in and activate your card
Follow the link in that email, set your password, and log in to the registry. Check that your personal details are correct, accept the legal statement, and select Activate Card.
No payment is requested — there's no fee to pay. Your digital card is typically available within a business day or two, and you download it from the registry rather than waiting for anything in the mail.

Why get a card when adult-use is legal?
Ohio's adult-use sales started in August 2024, so this is the fair question. The card still buys you several things it doesn't.
- You skip the adult-use excise tax. Medical purchases aren't subject to the 10% tax that recreational buyers pay on every transaction. For anyone purchasing regularly, that's the whole cost of certification back and then some.
- Higher purchase and possession limits than adult-use customers get.
- Access at 18 — adult-use requires 21. Patients between 18 and 21 have no other legal route.
- Dispensary discounts. Most Ohio dispensaries run medical patient pricing that adult-use customers don't get.
- A physician who knows what you're taking — including how cannabis sits alongside your other prescriptions, which is worth more than most people expect.
⚠️ The annual visit isn't optional
Ohio requires you to see your certifying physician at least once a year to keep the physician-patient relationship active. Your registration renews annually off the back of that visit, and the state won't renew a card without a current recommendation behind it.

What Ohio patients ask us most.
Where do I apply? I can't find the form.▾
There isn't one to find. Ohio patients can't self-register — your certifying physician creates your registry profile from their side, and the state emails you afterward to activate it. If you've been hunting for an application, that's why you can't find it. Book the visit and the rest follows.
Is the state really free now?▾
Yes. The Division of Cannabis Control eliminated the patient and caregiver registration fee effective May 15, 2024 — it had been $50 a year for patients and $25 for caregivers, and briefly dropped to a penny before being removed altogether. Renewals are free too. A lot of older guides still quote $50, so you may see it repeated elsewhere.
When does my card arrive in the mail?▾
It doesn't — Ohio issues digital cards through the registry. Once you've activated, your card is generally available within one to two business days and you download it from your account. Save it to your phone; that's what the dispensary checks.
Never got the activation email. What now?▾
Check spam first — that's usually it. If it still isn't there, the address on your profile may be wrong or mistyped. Give us a call and we can have it corrected. Don't start over with another appointment; that isn't the problem.
Can my regular doctor recommend it?▾
Only if they hold an active Certificate to Recommend from the State Medical Board of Ohio, which requires a full unrestricted license and a required education course. Several hundred Ohio physicians have one, but most primary care doctors don't. The Medical Board publishes a CTR search tool if you want to check yours.
Can I add a caregiver?▾
Yes, and it's free in Ohio like everything else on the state side. The key thing is that your physician has to link the caregiver to you in the registry — so ask at your appointment rather than trying to add them yourself afterward. Your caregiver then gets their own email to complete their registration, and consents to a background check. Note that your recommending physician can't also serve as your caregiver.
Do you certify patients under 18 in Ohio?▾
No — our Ohio practice is adults only, 18 and over. Ohio's program does allow minor patients through a caregiver, so if you're seeking care for a child, the State Medical Board's CTR search tool will help you find a practice that takes them.
Can I use my Ohio card in another state?▾
Some states recognize out-of-state medical cards for visiting patients and some don't, so check the rules where you're going. What's never allowed is carrying cannabis across state lines — that's federal, regardless of what either state permits.
