The Minnesota Patient Self-Evaluation, explained

A short form you fill out in your own registry account. It's what lets the dispensary release your medicine — and it takes a few minutes once you know where it lives.

What the PSE actually is

The Patient Self-Evaluation — you'll see it called the PSE or a "self-report" — is a questionnaire inside your Medical Cannabis Registry account. You enter your conditions and current medications, rate your symptoms, and note any side effects you've had from medical cannabis.

It does two jobs. Your dispensary's pharmacist reads it to decide which products and doses to recommend, and Minnesota uses the answers in aggregate for the outcome reports it publishes on conditions like chronic pain, PTSD, and sleep apnea. It is not a test, and there are no wrong answers — an honest "this isn't helping much" is more useful to your pharmacist than a score you think sounds good.

⚠️ You've probably read that it's required before every pickup

That was the old rule, and it's still all over older blog posts and printed patient guides. Under Minnesota's current requirements, the schedule is narrower:

  • Before your first purchase — required, no exceptions.
  • Quarterly through your first year — counted from the date of that first purchase, not from your enrollment date.
  • After one year — no more are required at all, unless the state tells you a new one is available.

That last part matters: if you do get a notice, complete it before your next pickup or the dispensary can't dispense.

When you'll need one

Notices show up two ways — in your registry account, and by email.

First purchase

Required. Complete it before your pharmacist consultation, not after — the consultation uses what you entered.

Year one, quarterly

Three more over your first year, timed off your first purchase date. You'll get an email and an in-account notice each time one opens up.

After year one

Nothing scheduled. You only fill one out if the state opens a new one for you.

Any open notice

Anything sitting open has to be finished before your next pickup. Your dispensary can see the notice in your account, so it's not something you can work around at the counter.

Log in to your registry account

Go to the Minnesota Medical Cannabis Registry and sign in with the email address on your account. Forgot the password? Use Forgot Your Password? on the login screen — the reset link is good for 24 hours, and your new password needs at least eight characters with a number or symbol.

Open Patient Self-Evaluation from Quick Links

On your patient home page, find the Quick Links area and choose Patient Self-Evaluation. This is the same place you'd go to view forms you've already submitted.

Choose Start Self-Report

Select Start Self-Report to begin a new form for your next pickup. View pulls up an earlier one if you just want to check what you said last time.

Add your conditions and medications

Use Add Medical Condition to enter your qualifying condition(s), then Add Medication Usage for everything you're currently taking — prescriptions, over-the-counter, supplements. Then continue.

Keep this current. Your pharmacist is checking it for interactions, and it's the reason we ask you to bring a medication list to your dispensary visits too.

Answer the symptom questions

These are required — the form won't submit without them, and you can't pick up medicine on an unsubmitted form. Answer for how you've actually been doing, not how you were at your certification visit.

Add side effects, then submit

Use Add Side Effect for anything you've noticed since starting medical cannabis, and leave it empty if there's nothing to report. Submit, and you're done — the form is immediately visible to your dispensary.

The state doesn't approve it — your dispensary does

This confuses a lot of people who submit a PSE and then sit waiting for an approval email. There isn't one. The Office of Cannabis Management doesn't review self-evaluations at all. Dispensary staff review and approve yours either before or at the time of pickup, so once you've submitted, the next step is simply your appointment.

Common questions

What Minnesota patients ask us about the PSE.

Can someone else complete it for me?

A registered caregiver can complete your PSE on your behalf. If you need help and don't have a caregiver, dispensary staff are required to assist you with a required self-evaluation and to review and approve it before dispensing.

I don't have email, or I can't do the form online.

Contact the Office of Cannabis Management directly to discuss other options — the program has alternatives for patients who can't complete a PSE electronically. Our team can also walk you through it over the phone if the screens are the problem rather than the access.

Will a bad self-evaluation get me removed from the program?

No. The PSE isn't a re-approval, and reporting that a product didn't help or that you had side effects doesn't put your enrollment at risk. It's clinical feedback that helps your pharmacist change the plan — which is the whole point of reporting it.

I submitted mine and nothing happened. Did it go through?

Probably yes. There's no confirmation email and no state approval step, so silence is normal. Log back in, open Patient Self-Evaluation, and use View — if your form is listed there, it's submitted and your dispensary can see it.

Do I need a new one if I switch dispensaries?

Not for the PSE itself — it lives in your state registry account, not with any one dispensary. But a new dispensary may want its own pharmacist consultation before it dispenses to you, so it's worth calling ahead before your first visit there.

Is the PSE the same as re-enrolling?

No, and it's worth keeping them straight. The PSE is about your medicine and happens inside your first year. Re-enrollment is about staying in the program and happens every three years, and it needs a new certification visit. See re-enrolling with the state.
