To regain access to your account, please follow the instructions below.

Navigate to your state’s patient login page.

Once you are on the login page, click the link for “Forgot Username or Password”

WEST VIRGINIA

Follow the instructions. Typically enter your email address you used to register with the state and request a reminder of your username. That will come right away to your inbox. Make sure you check your spam folder if you don’t see it. Make a note of your username (exactly as it looks). Return to the state’s login page, click on the link for “Forgot username/password” and enter your username to receive a link to reset your password. Passwords need to be 8 characters and include both upper and lower case letters, a number and a symbol such as ? or !. Please make sure you carefully make a note of your password. The state will then take you to the login where you can enter your username and new password carefully to login in.
