Scheduling your certification or renewal with Green Bridge Society only takes a few minutes — but a few small details on the intake form make all the difference. Here's what to know before you begin.

Our doctors call patients in the morning, afternoon, and evening — including weekends if your appointment is scheduled for a Friday.

Calls may come from a private, unknown, or spam-labeled number, so please keep your phone nearby and turned on during your window.

📋 Complete the Intake When You Schedule

You'll fill out a short intake form as part of the scheduling process. This must be completed before your certification can be issued.

Please double-check:

Last name & date of birth — must exactly match what your state's system has on file.

Phone number — this is the number your doctor will call.

Email address — this is where we send your confirmation and state instructions.

🔎 How Doctors Locate Your Record

Our doctors locate your profile in the state system using your last name and date of birth.

If that information doesn't match what the state has on file, we can't complete your certification until the state record is corrected.

✅ What Happens After the Call

If you qualify, your doctor submits the certification to your state's medical marijuana system right after the call. We then walk you through any remaining state portal steps — payment, address verification, card download.

What information do I need to fill out the intake form?

You'll need your full legal last name and date of birth (matching your state record exactly), a working phone number, an email address you have access to, and basic information about the medical condition you're seeking certification for. In Pennsylvania you'll also need your state Patient ID.

Why does my last name and date of birth have to match the state exactly?

Doctors search the state's medical marijuana system using these two fields to locate your patient profile. If the information you give Green Bridge Society doesn't match the state record character-for-character, the doctor can't find your account and the certification can't be submitted.

What if my name has changed and the state still has my old name?

Use whichever name is currently on your state record for the intake form, even if it's not your current legal name. Then update your name with your state's medical marijuana program separately. Most states require ID and supporting documentation for name changes.

What number will the doctor call from?

Doctor calls often come from private, unknown, or spam-labeled numbers because of HIPAA-compliant calling systems. Answer all calls during your scheduled window — even ones that look like spam.

Will the doctor call on weekends?

Yes, if your appointment is scheduled for a Friday or weekend window. Doctor calls run from 10 AM to 9 PM seven days a week.

What's the most common reason for delays?

The number-one cause of certification delays is a mismatch between the intake form and the state record — usually a typo, a married name not yet updated, or a date-of-birth digit transposition. Always double-check before submitting.

What if I miss the doctor's call?

Our office can see when calls are missed and we'll work with the doctor to try you again. If you know you're going to miss your window, contact us at (814) 360-5353 to reschedule.

How long does the doctor call take?

Most calls take 5-15 minutes. Our doctors don't rush you — they review your history, answer your questions, and complete the certification efficiently. New patient visits sometimes run a few minutes longer than renewals.

Can I pick a specific time, or just a window?

You pick a call window — Morning, Afternoon, or Evening — not an exact time. Your doctor will call you anytime within that window. This lets us serve more patients efficiently while still giving you flexibility about when to be available.
