Becoming a New York medical cannabis patient
New York has the shortest process of any state we work in. One telehealth visit, and you're registered — there's no application to file and nothing to wait for.
You're registered automatically
When your practitioner issues your certification, you're enrolled in New York's Medical Cannabis Program at the same moment. No separate application, no portal to visit, no state fee — and since March 2023, no plastic card either.
Your certification is your registration
This is the part that surprises people coming from other states. New York's Office of Cannabis Management stopped issuing physical Registry ID cards in March 2023. Instead, your registry ID number appears on the certification itself.
At a dispensary you present your certification along with a government-issued photo ID. That's the whole verification — nothing to activate, nothing in the post.
Any condition can qualify
New York removed its fixed list of qualifying conditions. Since the Marijuana Regulation and Taxation Act, a registered practitioner may certify a patient for any condition they believe can be treated with medical cannabis — it's clinical judgement rather than a checklist.
So "do I qualify?" is a conversation with the practitioner. Come ready to talk about your symptoms and what you've already tried.
The two steps
Genuinely two. There isn't a third.
Book your telehealth visit
Pick a time and complete the intake form. New York permits certification by telehealth, so this is by phone or video from home.
You'll need to be a New York resident and 18 or older to certify on your own. New York allows a broad range of clinicians to certify — physicians, nurse practitioners, physician assistants, dentists, podiatrists, and midwives — provided they're registered with the OCM and have completed the required training.
Your practitioner issues the certification — and you're registered
The certification is entered into the OCM's system and issued to you with your registry ID on it. Registration happens automatically at that moment; there's nothing further for you to submit.
Your practitioner also determines how long the certification runs and how much you're authorized for. Check the expiration date when you receive it — practitioners can set a shorter period where that's clinically appropriate, so don't assume the maximum.
Save a copy to your phone and keep a printed one somewhere sensible. That document is what gets you served.
Then go shopping
Bring your certification and a government-issued photo ID to any OCM-licensed medical dispensary. There are over 200 statewide, so there's likely one reasonably close wherever you are.
New York scales your allowance generously: registered patients can possess and purchase whichever is greater — the supply your practitioner authorized, or the standard adult-use threshold. Refills become available as you work through your current allocation.
Why certify when adult-use is legal?
New York's adult-use market opened in December 2022. The tax difference is the short answer.
Medical cannabis purchases are exempt from New York's cannabis taxes.

- Access at 18 — adult-use requires 21.
- Larger allowances, scaled to whichever is greater between your practitioner's authorization and the adult-use limit.
- Medical-only product lines that aren't on the adult-use menu.
- A practitioner who knows what you're taking, including how cannabis interacts with your other prescriptions.
What it costs
One payment. New York charges nothing.
⚠️ If you'll need a caregiver, mention it at the visit
New York caregivers register separately with the OCM and receive their own registry ID. To purchase on your behalf they'll need their photo ID, a copy of your certification, and their registry ID.
One quirk worth knowing for later: if you're ever issued a new certification, your caregiver has to re-register. If your existing certification is simply extended, their registration extends with it. Worth remembering at renewal time.

What New York patients ask us most.
When does my card arrive?▾
It doesn't. New York stopped issuing physical Registry ID cards in March 2023 — your registry ID is printed on your certification instead. If you're waiting on something in the mail, nothing is coming and nothing has gone wrong. Show the certification plus photo ID at the dispensary.

Possibly. New York allows physicians, nurse practitioners, physician assistants, dentists, podiatrists, and midwives to certify, provided they're registered with the OCM, licensed to prescribe controlled substances, and have completed the required course. Many practitioners haven't registered — worth asking yours, and we're here if the answer is no.
Can I grow my own?▾
New York permits home cultivation for certified patients and their caregivers aged 21 and over, within the state's limits on plant counts and how they're kept. Worth reading the OCM's own guidance before you start, since the rules on where plants can be grown are specific.
Do you certify patients under 18 in New York?▾
No — our New York practice is adults only, 18 and over. New York does allow minor patients, with a caregiver who must accompany them to the appointment and be approved by the OCM. If you're seeking care for a child, the OCM's patient pages are the place to start.
Can I use my certification in another state?▾
Some states recognize out-of-state medical patients and some don't, so check the rules where you're going. Carrying cannabis across state lines is prohibited under federal law regardless of what either state permits.
