Becoming a Maine medical cannabis patient
Maine has the most open medical cannabis program in the country. No list of qualifying conditions, no mandatory registration, and one telehealth visit is the whole process.
Maine has no qualifying conditions — at all
Maine repealed its list for adults in 2018 and for minors in 2022. A licensed provider may certify any patient they believe will benefit from medical cannabis. There's no list to match, and no such thing as a non-qualifying condition.
What that means in practice
In most states the first question is whether your diagnosis is on the list. In Maine there is no list, so the question is simply whether a provider believes cannabis will help you — their professional judgement, exercised at the visit.
That makes Maine one of the most accessible programs anywhere. It doesn't make certification automatic: it's still a clinical decision, so come prepared to talk about your symptoms and what you've already tried.
Your certification is enough. The card is optional.
Maine is one of the few states where registering with the state is genuinely a choice.
The written certification
Issued by your provider through Maine's mandatory online certification system, which produces a standard printed certification.
That document is legally sufficient to purchase from any licensed Maine dispensary. Most patients never need anything else.
The state registry ID card
Applied for separately through the Office of Cannabis Policy, with its own state fee.
Entirely voluntary for patients. Worth considering only if you'd prefer a wallet card, or if you're planning to act as a caregiver — which is a different matter, see below.
The two steps
There's no state processing in between, so this moves quickly.
Book your telehealth visit
Maine permits certification by telemedicine, so this is a video consultation from home. You'll need to be a Maine resident with a valid state ID or driver's licence, and 18 or over to certify on your own.
Maine allows physicians, nurse practitioners, and physician assistants in good standing to certify, provided they're registered to use the state's certification system.
Your provider issues the certification
Maine requires providers to use the state's online certification system — its use is mandatory, not optional — which produces a standard printed certification. You'll receive that document.
Keep it safe and keep a copy. Unlike states with registries and portals, in Maine that certification is your proof, so a lost original is genuinely inconvenient. Photo it on your phone as soon as you have it.
Then go to a dispensary
Bring your certification and your Maine ID to any licensed dispensary. There's no approval step to wait for and nothing to activate — the certification is valid from the moment it's issued.
⚠️ Caregivers do need a card — patients don't
This is the asymmetry to know about. Registration is voluntary for patients but mandatory for caregivers, who must hold a state card to act in that role. A background check fee applies on top of the card fee.
The exception is a parent, legal guardian, or person with legal custody of a minor patient, who is treated differently. If someone will be collecting on your behalf, factor in their registration rather than assuming Maine's relaxed approach extends to them.

Can someone else pick up for me?▾
Only if they're a registered caregiver, and unlike patients, caregivers must hold a state card — there's a card fee plus a background check fee. Parents, legal guardians, and those with legal custody of a minor patient are handled differently. Plan for that registration rather than assuming it works like the patient side.
Can I grow my own?▾
Maine permits home cultivation within set limits on flowering plants, immature plants, and seedlings. The rules on where and how plants must be kept are specific, so read the Office of Cannabis Policy's own guidance before you start rather than relying on a summary.
Do you certify patients under 18 in Maine?▾
