New patients looking to obtain a certification in PA, are required to submit a photo showing your qualified medical diagnosis. Records are not necessary. Only a picture of your qualified condition. If you are not sure what you have will work, please send it in with your submission and we will contact you. *The list of approved conditions is shown on the form below. New certifications cannot be completed by our doctor without this information.

If you suffer from undiagnosed anxiety or chronic pain, and would like to be evaluated by one of our physicians, please complete the appropriate form in our services tab above.

We do not invoice you for a new certification unless our doctor is going to certify you. If you are not certified for health reasons, we issue a full refund.

All new patients must register at the PADOH and create a patient registration account. If you’re having trouble with that, we are happy to take care of that step for you. You can find a link to the state’s website for patients in the services tab above.
