Renewing in Maryland
Maryland has two things that expire on very different schedules. Nearly every access problem here comes from mixing them up — so start by working out which one you're renewing.
Which one is expiring?
Almost always the first one. The second comes round so rarely people forget it exists.

In the sixth year, you'll also renew the registration itself on OneStop. That one's between you and the state, and we'll help you through the portal if it's giving you trouble.

Your provider files the new certification
It goes straight into your existing MCA profile, the same way as the first one. Nothing for you to submit, no fee to the state, and no approval queue.
Worth using the visit properly: a year is long enough for things to change, so talk about what's working and what isn't rather than treating it as a formality.
Carry on as normal
Once the certification is active you can purchase again. Dispensaries verify you against your registration, so there's no new card to collect and nothing to activate.

Different process, different place, and only occasionally.
Log in to OneStop
Sign in to the Maryland OneStop portal with the account you set up when you first registered. If you've lost access to that email address, sort it out before the renewal date rather than during it.
Update your details and pay the state fee
You'll upload a current photograph, confirm your Maryland residency, and pay the MCA's fee. The fee is reduced or waived for some patients — Maryland Medicaid participants in particular — so it's worth checking rather than assuming.
Keep your certification current alongside it
The two are independent. Renewing your registration doesn't extend your certification, and vice versa. If both happen to fall due around the same time, handle the certification with us and the registration on OneStop.
What renewal costs
⚠️ "My registration is fine, so why can't I buy?"
This is the Maryland conversation we have most. Patients check OneStop, see their registration active for another three years, and assume everything's in order — but it's the certification the dispensary needs, and that one runs out annually.
An active registration with an expired certification looks fine in the portal and doesn't work at the counter. If you've been turned away and the portal says you're registered, the certification is almost certainly why.
Common questions
What renewing Maryland patients ask us.
My OneStop account says I'm registered. Why was I refused at the dispensary?▾
Because registration and certification are separate. Registration lasts six years; the written certification lasts one, and it's the certification that authorizes purchases. An active registration with a lapsed certification is the most common reason a Maryland patient gets turned away. Book a renewal visit and it's resolved.
How early should I renew my certification?▾
A few weeks before the expiry date is plenty. There's no state processing queue in the way, so you don't need the long lead time other states demand — but do it before the date rather than after, since access stops the moment it lapses.
What happens if my certification expires?▾
You can't purchase as a medical patient until a new one is filed, but nothing else is lost — your registration stays intact and you don't start over. A renewal visit puts it right, usually the same day.
Do I need to do anything on OneStop at my annual renewal?▾
Not usually. Your provider files the new certification into your existing profile, so there's nothing for you to submit. The portal only comes back into it at your six-year registration renewal, or if your address or contact details have changed.
Can I switch to you from another provider?▾
Yes, easily. Your MCA registration is yours and doesn't move — any MCA-registered certifying provider can file your next certification into it. There's nothing to transfer and no state notification needed. Just book the renewal and mention you're coming from elsewhere.
How do I know when my six-year registration is up?▾
It's shown in your OneStop account under your registration details, and the MCA emails the address on your profile. That's a good reason to keep that email address current — six years is long enough for people to change providers, jobs, and inboxes.
Is it still worth renewing now that adult-use is legal?▾
For regular buyers, comfortably. Medical purchases are exempt from Maryland's cannabis sales and use tax, which rose to 12% for adult-use in July 2025 — so a patient spending a few hundred dollars a year recovers the $75 quickly. Patients also keep the higher possession limit, access from age 18, and medical-only products.
