Registering on the Michigan State Portal

After your certification visit, you'll register with the Michigan CRA portal to get your medical card. Here's exactly what to click, step by step.

ℹ️ Before you start: Online registration is for adult patients without a caregiver. If you're registering a minor or using a caregiver, Michigan requires a paper application instead — reach out and we'll walk you through it.

Choose "Patient Only Registration Application"

Once you're logged in to your state account, select the Patient Only Registration Application option to begin.

Read and accept the state's terms

Review the terms, then check the box confirming you've read and accepted them to continue.

Create your application

Click to create a new application. This opens the form you'll fill out next.

Enter your personal information

Fill in your name, address, and other requested details. Make sure your address matches your Michigan ID.

Enter your doctor's information

At the end of the application, you'll be asked for your certifying physician's details — including the MD number. Green Bridge provides your doctor's information after your visit, so keep that email handy for this step.

After you submit: The state reviews your application and issues your registry card. Wait for your official card before purchasing — and if you get stuck on any screen, we're happy to help.
