🇺🇸 Pennsylvania (PA)
Log in at padohmmp.custhelp.com. Make sure your name and date of birth match what you submitted on your intake form. If your certification is complete but you don’t see it in the portal, check to see if your new card was issued on your profile settings page.
PA Certification Guide🇺🇸 West Virginia (WV)
Go to mmcc.wv.gov. Use the same email address you provided to the doctor. If you’re having trouble uploading documents or navigating the system, we can walk you through it over the phone.
WV Certification Guide🇺🇸 Ohio (OH)
Log into ohiomedicalmarijuanaregistry.com. Your email must match your doctor's submission. After certifying, you’ll receive an email from the Board of Pharmacy to activate your registration and pay the $0.01 fee.
OH Certification Guide🇺🇸 Michigan (MI)
Visit aca-prod.accela.com/MIMM. You must apply as a patient without a caregiver to use the online system. After the doctor certifies you, log in to complete your application and upload your proof of residency.
MI Certification Guide🇺🇸 Arizona (AZ)
Arizona patients apply through the AZDHS portal. Once certified, you’ll receive an email with a link to register. The process includes uploading your photo, driver’s license, and payment online.
